About CTIC

TRANSITION

The results of transition planning enable the student to move to adulthood as independently as possible. The process utilizes the Individual Education Program (IEP) and involves a team planning process.
THE TEAM

The team includes the student, family, school, and community resources. Their role is to:

plan early

explore school and community options

foster self-advocacy

enhance skill development

provide experiences

connect services

WHEN DOES TRANSITION PLANNING START?

Minnesota legislation requires that transition planning begin during grade 9 for all students with an IEP. The IEP must document the individual’s transition needs and identify a plan for instruction and services.
TRANSITION PLANNING

includes student, family, school personnel and representatives of the necessary adult service agencies

assesses the student's preferences, current abilities, and future needs

focuses on career development, functional skills, and applied academics

links the school experience to adult life

reviews the progress and updates the plan annually

increases awareness of community participation opportunities

TRANSITION AREAS

Home/Daily Living:
Acquisition and awareness of skills needed to participate in life in the home
Community Participation:
Acquisition and awareness of skills needed to access community resources including transportation, people, public places and activities
Recreation & Leisure:
Acquisition and awareness of skills needed for activities during free time
Employment:
Acquisition and awareness of skills needed for employment
Post-Secondary Training & Education:
Acquisition and awareness of skills needed for Education and/or training
