About CTIC

TRANSITION

The results of transition planning enable the student to move to adulthood as independently as possible. The process utilizes the Individual Education Program (IEP) and involves a team planning process.

THE TEAM

The team includes the student, family, school, and community resources. Their role is to:

  • plan early

  • explore school and community options

  • foster self-advocacy

  • enhance skill development

  • provide experiences

  • connect services

WHEN DOES TRANSITION PLANNING START?

Minnesota legislation requires that transition planning begin during grade 9 for all students with an IEP. The IEP must document the individual’s transition needs and identify a plan for instruction and services.

TRANSITION PLANNING

  • includes student, family, school personnel and representatives of the necessary adult service agencies

  • assesses the student's preferences, current abilities, and future needs

  • focuses on career development, functional skills, and applied academics

  • links the school experience to adult life

  • reviews the progress and updates the plan annually

  • increases awareness of community participation opportunities

TRANSITION AREAS

Home/Daily Living:

Acquisition and awareness of skills needed to participate in life in the home

Community Participation:

Acquisition and awareness of skills needed to access community resources including transportation, people, public places and activities

Recreation & Leisure:

Acquisition and awareness of skills needed for activities during free time

Employment:

Acquisition and awareness of skills needed for employment

Post-Secondary Training & Education:

Acquisition and awareness of skills needed for Education and/or training
