When Does Transition Planning Start?
Minnesota legislation requires that transition planning begin by age 16 or grade 9 for all students with an IEP/IIIP. The IEP must document the individual’s transition needs and identify a plan for instruction and services. Transition Planning

Home/Daily Living: Necessary skills to participate in life in the home Community Participation: Skills needed to access community resources including transportation, people, public places and activities Recreation & Leisure: Acquisition of preferred activities during free time Employment: Acquisition of skills for employment or other meaningful adult activities Post-Education Training & Education: Education and/or training that occurs over a lifetime |
