About CTIC

Transition

The results of transition planning enable the student to move to adulthood as independently as possible. The process utilizes the Individual Education Plan (IEP/IIIP) and involves a team planning process.

The Team

The team includes the student, family, school, and community resources such as: employers, Rehabilitation Services, Center for Independent Living, Work Force Center, and other service providers. Their role is to:

  • plan early

  • explore school and community options

  • foster self-advocacy

  • enhance skill development

  • provide experiences

  • develop services

When Does Transition Planning Start?

Minnesota legislation requires that transition planning begin by age 16 or grade 9 for all students with an IEP/IIIP. The IEP must document the individual’s transition needs and identify a plan for instruction and services.

Transition Planning

  • includes student, family, school personnel and representatives of the necessary adult service agencies

  • evaluates the student's preferences, current abilities, and future needs

  • focuses on career development, functional skills, and applied academics

  • links the school experience with adult life

  • reviews the progress and updates the plan annually

  • increases results in community participation

Transition Areas

Home/Daily Living:

Necessary skills to participate in life in the home

Community Participation:

Skills needed to access community resources including transportation, people, public places and activities

Recreation & Leisure:

Acquisition of preferred activities during free time

Employment:

Acquisition of skills for employment or other meaningful adult activities

Post-Education Training & Education:

Education and/or training that occurs over a lifetime
