WHEN DOES TRANSITION PLANNING START?
Minnesota legislation requires that transition planning begin during grade 9 for all students with an IEP. The IEP must document the individual’s transition needs and identify a plan for instruction and services. TRANSITION PLANNING

Home/Daily Living: Acquisition and awareness of skills needed to participate in life in the home Community Participation: Acquisition and awareness of skills needed to access community resources including transportation, people, public places and activities Recreation & Leisure: Acquisition and awareness of skills needed for activities during free time Employment: Acquisition and awareness of skills needed for employment Post-Secondary Training & Education: Acquisition and awareness of skills needed for Education and/or training |
