The process utilizes the Individual Education Plan (IEP/IIIP) and involves a team planning process. The Team The team includes the student, family, school, and community resources such as: employers, Rehabilitation Services, Center for Independent Living, Work Force Center, and other service providers. Their role is to: plan early explore school and community options foster self-advocacy enhance skill development provide experiences develop services It's A Partnership HOME INDIVIDUAL COMMUNITY / SCHOOL When Does Transition Planning Start? Minnesota legislation requires that transition planning begin by age 16 or grade 9 for all students with an IEP/IIIP. The IEP must document the individual’s transition needs and identify a plan for instruction and services. Transition Planning includes student, family, school personnel and representatives of the necessary adult service agencies evaluates the student's preferences, current abilities, and future needs focuses on career development, functional skills, and applied academics links the school experience with adult life reviews the progress and updates the plan annually increases results in community participation Transition Areas Home/Daily Living: Necessary skills to participate in life in the home Community Participation: Skills needed to access community resources including transportation, people, public places and activities Recreation & Leisure: Acquisition of preferred activities during free time Employment: Acquisition of skills for employment or other meaningful adult activities Post-Education Training & Education: Education and/or training that occurs over a lifetime
