Statewide Testing Parent/Guardian Guide and Refusal Form

Minnesota Statutes, section 120B.31, subdivision 4a, requires school districts to include a form for parents and guardians to complete if they refuse to have their student participate in state-required assessments.  This information is also available in each school’s student handbook.
