Transition 2 Success
The services we offer are designed for students who have completed four years of high school and require continued academic and functional support as outlined in their Individualized Education Plan (IEP). We foster a collaborative partnership between parents, students, and community agencies to support the transition to adulthood, at no cost to the student or family. Our primary objective is to promote independence in all areas of students' lives, with a focus on the following key areas:
- Job Skills
- Independent Living
- Recreation and Leisure
- Home Living Skills
- Community Participation
In addition to the core program, students may also have access to related services, including school counseling, speech and language therapy, and/or occupational therapy. We offer transition planning to support students in returning to a general education setting or pursuing post-secondary options following graduation. This collaborative approach is integrated into each student's IEP to ensure personalized and effective support throughout the transition process.
